Registration
Visits at the International Center of Hearing and Speech may be scheduled by the patient personally at the registration desk or by phone, e-mail or by completing an Internet application form on www.ifps.org.pl. During the patient's stay at the Center information about the place of examination or consultation is communicated visually.
Each patient receives his own magnetic identity card with personal data (first name and surname) and the unique patient's ID number. That number is the patient's reference in the database of the Institute's Medical Information System. The same system functions also at the Institute's subsidiaries. Patients registered in the National Health Insurance System (National Health Fund - NFZ)
Patients visiting the Institute should have:
- referral from a doctor registered under the NFZ system (family doctor or specialist)
- up-to-date document confirming health insurance.
Patients outside the National Health Insurance System - NFZ
- should have referral from a doctor.
